This statement explains what happens to your information when you visit this website. It is separate from, and narrower than, the privacy information Smile Advisor provides to users of its screening service.

Smile Advisor is operated by The Smile Advisor Pty Ltd (ABN 98 688 903 246), based in Australia.
Scope of this statement
This statement covers this website (thesmileadvisor.com) only. The Smile Advisor service/app provides its own privacy information to its users; a full service privacy policy will be published separately.
What we collect on this website
We collect information in three places on this website: the contact form, our discovery survey, and our Standard 5 self-check.
Contact form. You submit your name, your organisation or facility (optional), your email address, and your message. We use this solely to respond to your enquiry. We don’t use it for marketing, and we never sell it. Your submission is sent to our team as an email (delivered via Google’s Gmail service) and is not stored in any database on this website.
Discovery survey. If you complete our survey at /research, your answers are stored securely in a database hosted in Australia. We don’t ask for your name or facility, and responses are reported only in aggregate. If you choose to leave contact details for a follow-up conversation, we use them only to arrange that conversation. They aren’t shared with anyone and aren’t added to any mailing list.
Standard 5 self-check. If you complete the self-check at /standard-5-oral-health-check, your answers are stored securely in a database hosted in Australia. We don’t ask for your name or facility, and responses are reported only in aggregate, never by facility. There’s no contact field on this one.
